Job Description:

We are seeking an experienced and detail-oriented Associate Case Manager to join our team. In this role, you will collaborate with the case management team to provide support and ensure seamless patient transitions. You will assist with documentation, communication, and ongoing case management support, ensuring all activities comply with relevant laws, regulations, and company policies.

Key Responsibilities:

  • Provide case management support for all patients, ensuring effective and efficient care coordination.
  • Work closely with the case management team to provide ongoing support for patients' needs.
  • Assist with site communication and documentation requirements to ensure accurate and timely reporting.
  • Help case managers with updating patient transitions within the system.
  • Ensure compliance with all legal, regulatory, and client-specific policies and guidelines, including HIPAA regulations.
  • Maintain ongoing communication with the internal team regarding patient transitions and case updates.
  • Some travel may be required.

Qualifications:

  • Minimum of 5 years of experience in the pharmaceutical, healthcare, or related industry.
  • Strong attention to detail and ability to manage multiple tasks.
  • Excellent communication skills, both written and verbal.
  • Knowledge of compliance and HIPAA regulations.
  • Ability to work effectively in a team environment.
